What's the Difference Between a Doula and a Midwife?
If you're expecting a baby, you've probably come across both the terms doula and midwife and wondered what the difference is.
It's one of the questions I'm asked most often as a birth doula, and it's an important one because while both roles support families through pregnancy and birth, they do so in very different ways.
The simplest way to think about it is this:
A midwife provides your medical care.
A doula provides emotional, physical and practical support.
Both roles are valuable, and many families find that having both a midwife and a doula helps them feel more supported throughout their pregnancy and birth journey.
What Does a Midwife Do?
A midwife is a qualified healthcare professional responsible for the medical care of you and your baby during pregnancy, labour, birth and the postnatal period.
Your midwife will:
Monitor your pregnancy and your baby's wellbeing
Carry out antenatal appointments and health checks
Support you medically during labour and birth
Discuss pain relief options
Recognise and manage complications
Provide care after your baby is born
Midwives play an essential role in maternity care and are there to help keep both you and your baby safe.
Depending on where you choose to give birth and how your local maternity service is organised, you may see several different midwives throughout your pregnancy and labour.
What Does a Doula Do?
A doula is a non-medical professional who provides continuous support during pregnancy, birth and the postnatal period.
We don't perform clinical assessments, offer medical advice or deliver babies. Instead, our role is centred around supporting you emotionally, physically and practically.
As a birth doula, I support families by:
Helping you explore your options and make informed decisions
Supporting your birth preferences without judgement
Providing reassurance and encouragement throughout labour
Using comfort measures such as massage, movement, breathing techniques and positioning
Supporting birth partners so they feel calm, involved and confident
Offering continuity of support before, during and after birth
Creating a safe space where you feel heard, respected and supported

Do You Need a Doula If You Have a Midwife?
Many people assume that because they already have a midwife, they don't need a doula.
The reality is that the two roles complement each other.
Your midwife's primary focus is your clinical care and your baby's wellbeing. Your doula's focus is your overall experience.
One of the biggest benefits of working with a doula is continuity. While healthcare professionals may change shifts or have responsibility for several families at once, your doula remains a familiar and consistent presence throughout your pregnancy and birth.
Research has shown that continuous support during labour can contribute to a more positive birth experience and help parents feel more confident and supported throughout the process.
Can a Doula Replace a Midwife?
No.
Midwives are trained healthcare professionals who provide the medical expertise needed to care for you and your baby safely.
Doulas provide non-medical support that complements the care you receive from your maternity team.
When doulas and midwives work alongside one another, families often benefit from both professional medical care and dedicated emotional and practical support.
Is a Doula Right for You?
Every family is different, and there's no right or wrong choice.
Some people feel completely comfortable relying on their maternity team alone. Others would like additional support throughout pregnancy, labour and early parenthood.
